About the Opportunity:
We are seeking skilled and enthusiastic individuals to appoint
to Consultant Physician (Geriatrics) positions with the East
Metropolitan Health Service.
Permanent, Fixed-Term, and Casual
positions are available working Full-Time and Part-Time hours.
Fixed-Term and Casual appointments carry the possibility of
extension and/or permanency.
In this position, the successful applicant/s will lead the
Community Health in a Virtual Environment (Co-HIVE)
multidisciplinary team to provide specialist services to patients,
providing medical support and management as required, to patients
enrolled in the service.
Key aspects of the role include:
The promotion of patient safety and quality of care.
Provides leadership, orientation, training, supervision and
education, where relevant, for doctors in training and other
health workers.
Working in collaboration with the Head/Deputy Head of
Department and other Consultants works to achieve national,
state and EMHS performance standards and targets.
Working within the scope of clinical practice as defined
and recommended by the EMHS Area Medical Credentialing
Committee.

About the East Metropolitan Health Service &
Co-HIVE:
The East Metropolitan Health Service (EMHS) comprises an
extensive hospital and health service network that aims to maintain
and improve the health and wellbeing of more than 828,000 Western
Australians in its catchment area.
Our hospital network works together to provide a combination of
tertiary, secondary and specialist health care services including
emergency and critical care, state trauma, elective and emergency
surgery, general medical, mental health, inpatient and outpatient
services, aged care, palliative care, rehabilitation and women's,
children's and neonates services.
The EMHS consists of Royal Perth Hospital; Mt Lawley Hospital;
Armadale Health Service; Bentley Health Service; Kalamunda
Hospital; St John of God Midland Public Hospital and a range of
community and population health services, such as the Community
Health in a Virtual Workplace (Co-HIVE) Aged Care
service.
The Community Health in a Virtual Environment (Co-HIVE) Aged
Care service, a geriatrician-led virtual healthcare initiative




designed for individuals residing in Residential Aged Care
Facilities (RACFs) in Western Australia.
Co-HIVE provides
comprehensive, high-quality, and coordinated clinical care to older
adults, prioritising the most suitable setting for their needs.
Co-HIVE aims to empower RACFs to deliver quality care within the
comfort of residents' homes.
The service aims to bridge
communication gaps and enhance care coordination between community
and tertiary healthcare providers, fostering a seamless healthcare
experience.
The Co-HIVE Team comprises specialised clinicians, including
consultant Geriatricians, Palliative Care Physicians, Older Adult
Psychiatrists, and Clinical Nurses.
Co-HIVE is committed to facilitate improved healthcare outcomes
by promoting collaboration and communication among healthcare
providers, ensuring that older adults receive the most appropriate
care in the setting that best suits their needs.
