Position Summary
The Support Worker will provide Community Access support to clients in accordance with their individual NDIS plans.
The role focuses on supporting clients to safely access the community, participate in meaningful activities, attend appointments, and maintain their independence and wellbeing.
Key Responsibilities
Provide person-centred Community Access support in line with each client's NDIS goals and support planSupport clients to attend appointments, social activities, shopping, recreational activities, and other community-based activitiesAssist clients to safely access and participate in the community while promoting independence and choiceProvide transportation and/or travel assistance as required for community access and appointmentsEncourage clients to develop and maintain social connections and participate in activities that align with their interests and goalsProvide appropriate assistance with daily living tasks where required during community-based supportsMonitor and support client safety and wellbeing while in the communityMaintain accurate progress notes and records in accordance with Point Care requirementsCommunicate effectively with clients, families, support coordinators, and other members of the care teamFollow individual support plans, risk management strategies, health and safety requirements, and organisational policiesMaintain qualified boundaries and uphold client privacy, dignity, rights, and choiceParticipate in ongoing training and professional development to maintain compliance and provide high-quality support
RequirementsKey Requirements
Minimum of a High School Certificate or equivalentMust hold a valid Blue Card, Yellow Card, First Aid and CPRCurrent Police Check issued within the past 3 monthsHold a valid Open Driver's Licence.
An international/overseas driver's licence may be accepted for eligible temporary visa holders, subject to Queensland licensing requirementsHave a reliable, registered vehicle with current comprehensive car insuranceCertificate III/IV in Individual Support (Disability, Aged Care, or related field), or willingness to work towards completion - proof of enrolment required before commencementPrevious paid or lived experience supporting people with disabilityMust be currently located in Hervey Bay or surrounding areas at the time of applicationExperience providing Community Access support, including assisting clients with appointments, outings, shopping, social activities, and community participationStrong understanding of person-centred support, client choice, independence, and NDIS goalsStrong understanding of the NDIS and its Practice StandardsSound knowledge of health and safety requirements and duty of care obligationsAbility to support a diverse range of clients with different abilities, backgrounds, communication styles, and support needsStrong communication and interpersonal skills, with the ability to build positive and professional relationships with clients and their support networksGood organisational skills and the ability to maintain accurate progress notes and support recordsBasic computer literacy, including the ability to use Microsoft Office and digital support systemsWillingness to complete ongoing mandatory training and professional development as required by Point Care
