We are seeking an experienced and motivated Support Coordinator to join our team.

This Position requires an Individual who has:

- Experience providing NDIS Support Coordination/Recovery Coaching Services or Similar Role
- Support participants to understand and implement their NDIS plans
- Assist participants in building capacity, independence, and decision‑making skills
- Coordinate and connect participants with appropriate service providers




- Monitor supports and ensure services align with participant goals and funding
- A Solid understanding of the NDIS and its Importance to People with a Disability and their Support Networks
- Demonstrated experience with Facilitating Multi‑Disciplinary Coordinated Planning and Develop Collaborative Relationships and Networks between Service Providers
- Maintaining an Up‑To‑Date knowledge of the NDIS Systems and Practices
- Engaging with NDIA Staff and LAC’s
- Liaising with the Participant, Family or Representative on Supporting the Participant with Achieving their Goals
- Preparing for Plan Reviews and Attending with the Participant, Family or Representative
- Maintain accurate and compliant documentation in line with NDIS requirements
