Your Side Australia is seeking a Care Partner to join our aged care team on a full time basis. You will coordinate care for clients on Support at Home packages, visiting homes, developing care plans, and managing budgets with clinical support from RN qualifications.
Hybrid role with office base in Chatswood and community visits; a valid driver's licence and access to a vehicle are required. You will work to improve wellbeing and independence for older Australians.
