Administration Officer - Community Assessment Response Team (CART)

Across multiple sites Clayton, Dandenong, Doveton, Berwick, Pakenham, and Cranbourne

The role is accountable for the effective and efficient day to day administrative operations across the CART program.

Intensive /Proactive /Primary streams.

About the Role

Join our team as an Administration Officer on a full time /part-timebasis 0800-1630 Monday to Friday, ongoing to work across multiple sites.

- We are looking for a talented Administration officer to support our CART Program this includes clinics (specialising in Chronic Disease) and Post Acute Care that supports the most vulnerable people in our community.

This role is integral in supporting the client's experience with CART,operatesfrom a person - centred care framework and aligns with the social model of health.

Financial accountability for client services-accuracy entering client diaries,



following up outstanding accounts,identify, resolve discrepancies with financialservicesandmaintainaccuratedata processing to support reconciliation processes

Audit KPI and data entry to ensure integrity of data submitted to Department of Health

About Us

TheCART Intensive Support Team – Post Acute Care (PAC)provides short-term support to people recovering after a hospital or Emergency Department visit. Our goal is to help clients recover safely at home, regain independence, and connect with ongoing community services when needed.

PAC is part of the broaderCART Intensive Support Team, which supports people living with chronic health conditions and/or complex psychosocial needswhofrequentlypresent tohospital or are at risk of hospitalisation.Through education, care coordination, and practical support, we empower clients to better manage their health, improve wellbeing, and reduce avoidable hospital admissions.

About You

You are a dynamic Administration Officer with a genuine passion and interest in this unique opportunity

Proficient in a range of IT applications and able to adapt to new systems

Excellent interpersonal and organisationaland time managementskills with an ability to liaise professionally with people at all levels

Excellent communication skills both written and verbal

Able to work flexibly within a larger team and across multiple sites

What You Need:

- Proficient in Microsoft Office Suite
- Valid Driver's licence
- Current Working with Children’s Check
- Current Police Check
- Accounts support & record keepingexperience
- Experience with Oracle fusion
- Relevant qualifications in Administration or Office Management
- Certificate in Medical Administration
- PreviousCommunity Administration experience
- Knowledge of HMS/Harrison, SMR, IPM and EMR
