About the role

The Paediatric HITH/Day Stay coordinator is responsible for leadership, governance and oversight of the Paediatric HITH/PAC and Day Stay service.The coordinator has day to day operational responsibility of the services by using clinical knowledge and leadership experience/skills.This advanced nurse role accepts accountability for the delivery of nursing care and patient experiencewithin the framework and values of Barwon Health. The HITH/Daystay coordination role is responsiblefor the productive and effective management of human, financial and material resources (including supplyordering and pharmacy), leads local strategic initiatives, and constantly re-evaluates local protocols andpatient care practices to ensure best practice.

Key responsibilities

- Time management, ability to undertake active communication with stakeholders both verbally andwritten, including patient’s family/carer in person and via phone/videoconference
- Expertise in delivering hospital level care in the home with appropriate clinical skills
- Critical thinking, analytic and problem-solving skills
- Provide Clinical education and mentoring of staff
- Scheduling/coordinating clinical contacts by the multidisciplinary team
- Provide clinical leadership to staff in the provision of clinical care including advanced wound care, specialized paediatric care, procedural sedation, routine infusions, and eating disorder management.
- Work as an advanced member of the multidisciplinary team, with expert communication skills
- Management of unexpected absenteeism and reallocation of clinical care to ensure service provision,other staff issues, incidents,



and accidents.
- Support the facilitation of care progression meetings and team planning meeting withing the service
- Reporting and providing initial local responses to reportable incidents.
- Constant re-evaluation of protocols and practices and patients care to ensure best practice.
- Supervise and coordinate patient care plans, ensuring delivered to applicable standards, as well as maintaining competence and educational needs of staff/team and ither nurses, and delegates asappropriate.
- Subspecialty coordination (eg, Chemotherapy)
- Service planning, personnel management with appropriate reference or escalation to manager.

What we’re looking for

- Current Victorian Drivers license
- Advanced experience in Paediatric nursing
- Ability to work autonomously and work within MDT
