Role Summary

About the role:

An exciting opportunity exists for a passionate and patient-centred Grade 2 Occupational Therapist to join Western Health at Home.

This role delivers Occupational Therapy services across the GEM at Home and Rapid Allied Health (RAH) programs, supporting patients to remain safe, independent and well within their own homes. Working within a multidisciplinary team, you will provide evidence-based assessment and intervention while contributing to innovative models of care that help prevent avoidable hospital presentations and admissions.

This unique opportunity offers a varied caseload across hospital substitution, aged care and rapid community response services.

Key responsibilities:

- Deliver comprehensive Occupational Therapy assessments and interventions in the home environment.
- Develop person-centred treatment plans that maximise safety, independence and functional outcomes.
- Provide rapid-response support to prevent avoidable Emergency Department presentations and hospital admissions.
- Collaborate with medical, nursing and allied health teams to coordinate high-quality care.
- Contribute to quality improvement, service development and supervision of students as required.

About you:

- Current AHPRA registration as an Occupational Therapist.
- Minimum two years' clinical Occupational Therapy experience.
- Experience in community, rehabilitation and/or aged care settings.
- Strong clinical reasoning, assessment and problem-solving skills.
- Excellent communication skills and ability to work effectively within a multidisciplinary team.
- Current Victorian Driver Licence.
