Fernlea Community Care in Pakenham is seeking a Client Engagement Officer to connect Fernlea with the aged‑care network and convert referrals into active clients. This role focuses on building relationships with assessors, package managers and health providers, and promoting Fernlea services across CHSP and Support at Home funding pathways.
You will identify referral opportunities, follow up promptly, maintain accurate reporting, and support client onboarding and service setup.
