Pay: (IB68 - IB72) $38.14 - $40.52 per hour / (YP2 - YP9) $42.61 - $54.41 per hour

About the Role
Grampians Health Horsham is a 20 bed sub-acute facility that provides admitted rehabilitation, geriatric evaluation and management (GEM), transition and palliative care services within a multidisciplinary team and patient focused environment.
Work Schedule
7‑day, 24‑hour rotating roster including day, afternoon, and night shifts. Permanent full‑time, part‑time and casual positions available.
Key Responsibilities
- Deliver individualised nursing care as part of a multi‑disciplinary team
- Liaise and effectively communicate with relevant medical, nursing and allied health team members
- Promote evidence‑based practice that has a problem‑solving approach
- Encourage patients, carers and families to participate in all stages of care
- When appropriate, supervise and direct clinical practice of junior Registered Nurses, Enrolled Nurses, and undergraduate nursing students and provide feedback
Skills & Experience
- Registered Nurse with current AHPRA registration
- Excellent communication and problem‑solving skills
- Knowledge of contemporary nursing theory and practice in relation to the care of sub‑acute patients/clients
- Thorough understanding of quality systems and health services accreditation
