Support Coordination is about far more than implementing an NDIS plan. It's about helping people understand their options, make informed decisions, build the right support network and move towards the life they want to live.
We're building a team of experienced Support Coordinators supporting participants across the Sunshine Coast and Moreton Bay regions, with growth plans into Aged Care and DVA. THE ROLE
This is more than a coordination role. It's an opportunity to become a trusted partner in someone's life. Working with your own caseload, you'll support participants to understand and implement their NDIS plans, navigate the disability sector, connect with the right supports and build the confidence to achieve their goals. No two days are the same, and that's exactly how we like it.
WHAT YOU'LL DO
- Support participants to understand and implement their NDIS plans.
- Help participants navigate the disability sector and connect with the right supports.
- Build participants' confidence and capacity to achieve their goals.
- Work closely with families, providers and community services to ensure the right support at the right time.
- Build genuine, trust-based relationships, not transactional ones.

As an established, registered NDIS provider, we've had the privilege of working alongside some truly outstanding Support Coordinators. We've also learnt something important. While billable hours are part of every sustainable business, they should never become the purpose of the role. The best Support Coordinators aren't remembered because they achieved a utilisation target. They're remembered because they answered the phone, followed through on their commitments, communicated openly and earned the trust of the people they supported.

WHAT YOU'LL BRING
- Experience in Support Coordination or a similar case management role, with a solid understanding of the NDIS.
- A genuine enjoyment of building relationships, solving problems and advocating for people when it matters.
- Strong communication skills and ownership of your work.
- A commitment to a consistently high standard of service, built on trust rather than transactions.

ESSENTIAL REQUIREMENTS
To be successful in this role, you'll have:
- Experience as an NDIS Support Coordinator or a similar case management role
- A relevant qualification in Community Services, Disability, Social Work, Human Services or a related discipline
- Current NDIS Worker Screening Check
- Current Blue Card
- Current First Aid and CPR Certificate
- Current Australian Driver Licence and access to a reliable vehicle
- The right to work in Australia
- Aged Care and DVA sector experience will be highly regarded as well.
