About Forever Care Disability Centre (FCDC)

At FCDC, we believe in person-centred care that goes beyond filling shifts; we focus on building a comprehensive support system tailored to the unique goals and needs of each participant.

We are currently seeking a compassionate, reliable and dedicated female Support Worker to join the support team for a participant residing in St Leonards / North Sydney.

The Opportunity

We are looking for a reliable professional to provide safe, respectful and high-quality support. Please review the specific requirements for this role below:

Shift Times: Shifts commence at 6:00 am. Exact finishing times and shift lengths will be confirmed through the approved roster.

Type of Support: Daily living support, personal care, domestic assistance, community access, transport, travel support, shopping and grocery support, health and medication follow-up, respite, complex disability support, high-care support and hospital discharge support where required.

Required Availability: Monday, Tuesday, Wednesday, Thursday, Friday, Saturday and Sunday. Applicants should clearly state which days they are available.

Personal Care Requirements: Assistance with showering, dressing, grooming, toileting, hygiene routines, bowel-care support and awareness, catheter and catheter-bag awareness, skin care and pressure-area awareness, meal assistance, mobility support, bed positioning and repositioning, and morning routines.

High-Care or Complex Support Requirements: Electric wheelchair support, transfer assistance, hoist transfers, slide-board transfers, two-worker transfers, bed-to-chair transfers, chair-to-shower-chair transfers, manual handling and falls-risk support.

The participant does not have identified behaviour-support needs.

Essential Criteria

Required Experience: A minimum of one year of disability support, aged care or similar care experience.

Preferred Experience:



Experience supporting participants with complex physical disabilities, high-care needs, personal care, hoist transfers, slide-board transfers, two-worker transfers, wheelchair support, manual handling, continence support, catheter awareness, pressure-area care and falls-risk support.

Transport Requirements: The worker may be required to drive the participant’s vehicle for community access, appointments, shopping and approved activities.

Licence / Car Requirements: A current driver licence is preferred. A personal vehicle is not required. A participant or company vehicle may be available where approved.

The worker must be willing to complete participant-specific training, buddy shifts and competency checks before commencing independent shifts.

Professional Expectations

Documentation Expectations: Commitment to providing clear, accurate, factual and objective shift notes, progress notes and incident reports.

App Usage Expectations: Ability to use FCDC systems for viewing rosters, clocking in and out and completing required documentation.

Reliability Expectations: A proven record of punctuality, reliability and commitment to assigned shifts and agreed roster patterns.

This is a high-care and higher-risk support role involving personal care, complex physical support, manual handling, falls risk and continence-related support. Workers must follow all approved support instructions, complete required training and report any risks or concerns immediately.
