Adelaide Primary Health Network (Adelaide PHN) is an independent, not for profit, primary health care organisation based in Adelaide. Job Description
The Role
In this exciting role you will lead population health needs assessments within the Adelaide Primary Health Network region,design of effective solutions, based on evidence and intelligence,andenablebest practiceevaluationthroughscopingandshowcasingfor a range of activities.
As a member of a dynamic and talented Planning and Performance team, the Planning and Design Coordinator will:
Lead planning and design of Adelaide PHN programs and activities in line with strategic and operational requirements.
Support the delivery of outputs,activitiesand projects in line with the Adelaide PHN Commissioning Framework and organisational accountabilities.
Apply design methodologies (including person centred and co-design) and developdesign summary andlogicdocumentationto ensure programs and activities are informed by evidence and evaluation outcomes.
Support evaluation planning, design and methodsincluding the alignment of primary care services and program metrics.
Contribute to collection analysis,disseminationand use of information for the purposes of design quality improvement andtheshowcasing,publicationand communication of evaluations.
Support collaborative and integrated approaches through partnership and consultation for the purposes of design andevaluation.
This is a full-time12 month fixed-term contract. Future employment is subject to ongoing funding.

Desired Skills and Experience
About You
To be successful in this roleyou’llneed:
Relevant tertiary qualifications.
Significant experienceworking in health care and/or public health.
Demonstrated experience in health planning, design, and/or development of activities,processesand services.
Demonstrated capability to apply design methodologies (including person-centred and co-design) to meet identified healthand serviceneeds.
Proven experience developing and conducting reviews and evaluations of health and community related projects.
Deep knowledge and understanding of the Australian Health System including new and emerging models of care and evidence-based approaches.
Proven ability to build andmaintainconstructive working relationships to engage and collaborate with a wide range of stakeholders and communities.
Demonstrated skills and experience in project management.
