As a key member of the ATSICHS Brisbane team,Contributing to excellentpatient care theCareCoordinatorNursesupports patients with Chronic Illness. TheCareCoordinatoris responsible forthe provision of effective service coordinationworking closely withclients with the aim to achieve improved health outcomes for all Aboriginal and Torres Strait Islander people.
Duties
- Provide care coordination toassistin the provision of chronic disease management.
- Establish andmaintainrelationships with key external stakeholders conducive to holistic health and social support needs.
- Foster a culture of continuous improvement and innovation within thePrimary Health Support Services, integrating culturally safe practices.
- Ensure effective implementation and maintenance of ATSICHS Policy and procedures in line with AGPAL Accreditation Standards.

- Starting Salary of $86,617per annum (based on qualifications and experience).

Required
Full un-restricted RegisteredNurse AHPRA registration.
Previousdemonstrable experience in relevant nursing field.
Demonstrated ability to meet targets and performance outcomes.
Ability to support a continuous improvement model for achieving outcomes.
Decisive listening,influencingand persuasive skills.
National Police Certificate.
DrivingLicence.
AHPRA Registration.
Evidence of Work Rights.
Preferred
Minimum two (2) years’ experience in a primary health setting.
Previousexperience of working with Aboriginal and Torres Strait Islander clients.
Previousexperience in a multidisciplinary health team.
Previousexperience in chronic disease management.
