Role Overview
An opportunity is available for a dedicatedCase Worker Targeted Care Packagesto support children and young people transitioning from Residential or Home-Based Care into environments where their needs can be better met and coordinate the daily operations of this vital program.
In this role you will work directly with the children and young person’s families, carers, and key stakeholders to facilitate change that supports the achievement of each child or young person’s case plan and the outcomes identified for their ongoing stability, safety and wellbeing.
This is a Permanent Full-time position based at our Mildura office location.
Key Responsibilities
- Provide case management and placement support functions in line with the case plan for children/young people.
- Coordinate care teams of diverse service providers and supports required to sustain children/young people’s placements and their families to achieve their goals with regards to health, wellbeing, education and employment,
- Support the caregivers of children/young people in order to sustain healthy, therapeutic placements for the child/young person in their care,
- Provide support in preparation for schooling to Aboriginal children and families to complement kindergarten,
- Directly engage with the child, their family, carers and other members of the care team in a supportive, inclusive and respectful manner,
- Work collaboratively with internal partners to ensure streamlined intake, assessment and referral processes.
Skills, Knowledge and Experience
- Strong communication and teamwork skills, with a caring, calm and respectful approach,
- An understanding of the local Aboriginal community, family dynamics, and issues faced within the local community,
- An understanding of transgenerationaltrauma, community trauma and the adverse effects that underpins the work with the families,
- Experience in the case management of complex children, youth and families and knowledge of the Residential Care, Out of Home Care system and Child Protection statutory requirements.
- Diploma of Community Services or a related discipline.

- Current National Police Check.
- Current Employee Working with Children Check
- Current Driver's Licence
