The Role

We are seeking a compassionate and motivated Care Partner to join our growing Support at Home team. Reporting to the Support at Home Manager, you will manage a caseload of clients and work closely with families, support workers, and healthcare professionals to ensure clients are living their best lives and achieving their individual goals with the highest level of wellness while navigating the evolving Support at Home program and proactively identifying solutions to emerging challenges. You will play a key role in maintaining compliance with Aged Care Quality Standards and Support at Home program requirements.

About You

You are compassionate,



organised and confident in building relationships with clients and stakeholders. You bring a strong understanding of aged care, and preferably the Support at Home program, or transferrable case management or clinical experience, along with a commitment to high quality service delivery.

Key Responsibilities

- Manage a caseload of Support at Home clients
- Conduct home visits, assessments, and care planning
- Develop and review person-centred care plans
- Coordinate services to support safe, independent living
- Monitor client wellbeing and respond to changing needs of clients
- Manage feedback, concerns and service issues
- Collaborate with internal and external stakeholders.

What You Need

- Minimum qualification of Certificate IV in Ageing and Disability or preferable a nursing or other clinical care qualification
- Strong administrative and documentation skills
- Excellent verbal and written communication skillsCurrent Victorian driver's licence (a fully maintained company vehicle will be provided)
- Current police check or be willing to obtain one
- Minimum of two COVID vaccinations in line with company policy
- Australian permanent residency is required for this position
- A clear police record is a mandatory requirement of this role and you must not be listed on the Aged Care Banning Orders
