About the role
We are seeking a dedicated and well-connected Client Service Manager to join our team in Western Sydney on a full-time basis. This is an outward-facing role: alongside overseeing client services and leading a team of support workers, you will grow Crescent Care's referral networks by proactively reaching out to organisations such as the Department of Communities and Justice (DCJ), hospitals, parole officers, Corrective Services and other community agencies, and by representing us at networking and industry events. You will ensure every person we support receives exceptional, tailored care, maintaining Crescent Care's reputation for excellence and our unwavering commitment to client wellbeing.
Key responsibilities
- Proactively reach out to and build referral relationships with organisations such as the Department of Communities and Justice (DCJ), hospitals, parole officers, Corrective Services and other community agencies
- Represent Crescent Care at networking events, interagency meetings and community forums to raise our profile and generate new referrals
- Manage and coordinate daily client support activities, ensuring all service users receive personalised care aligned with their individual care plans and goals
- Supervise and mentor a team of support workers, providing guidance, training and performance management to maintain service quality standards
- Conduct regular assessments of client needs and preferences, updating care plans as necessary to reflect changing circumstances
- Build and maintain strong relationships with clients, families and carers, acting as a primary point of contact for concerns, feedback and queries
- Ensure compliance with all relevant legislation, regulations and organisational policies, including aged care and disability support standards
- Liaise with external stakeholders including healthcare professionals, case managers and community agencies to coordinate holistic support for clients
- Monitor and document client progress, maintaining comprehensive and accurate records in accordance with statutory requirements
- Identify and elevate safeguarding concerns promptly, ensuring client safety and wellbeing remain paramount
- Support the development and implementation of initiatives that enhance client engagement, dignity and social inclusion
- Contribute to team meetings and quality improvement processes to continuously enhance service delivery
What we're looking for
- A minimum of 2 years' experience in aged care, disability support or community services management (essential)
- Demonstrated leadership and people management experience, with the ability to motivate and develop a team
- Confidence in stakeholder engagement and business development, with the ability to build and maintain referral networks across government, health, justice and community organisations
- Strong knowledge of aged care and disability support regulations,
standards and best practice in Australia (or willingness to rapidly develop this knowledge)
- Excellent communication and interpersonal skills with the ability to engage effectively with clients, families, staff and external partners
- A compassionate, person-centred approach to care with genuine commitment to supporting vulnerable populations
- Proficiency in health and social care information systems and electronic record-keeping
- Strong organisational and time management capabilities with ability to manage competing priorities
- Current or willingness to obtain relevant qualifications such as a Diploma in Aged Care or Community Services
- A valid driver's licence and willingness to travel between locations if required
- Working With Children Check and/or Working With Vulnerable People clearance, or willingness to obtain
- Preferred: Experience in service coordination, care planning or individual support planning within an NDIS or aged care context
