We are looking for an experienced NDIS Support Coordinator / Psychosocial Recovery Coach to join our team in Norwest and work alongside NDIS participants to turn their goals and funded supports into real-world outcomes.

This role could be a great fit for someone who understands the NDIS, is confident working with complexity, and brings a genuine recovery-oriented, strengths-based and person-centred approach to their work.

What you will be doing:

As a Support Coordinator, you will work alongside participants, families, carers and key stakeholders to:

- Help participants understand, implement and make the most of their NDIS plans
- Coordinate and connect participants with the right NDIS, mainstream and community supports
- Build strong provider relationships and ensure supports are working together effectively
- Support participants to build capacity, independence, social and community participation
- Monitor plan utilisation, funding and participant outcomes
- Identify challenges and work proactively towards practical solutions
- Maintain accurate documentation and manage caseloads and billable coordination hours effectively

As a Psychosocial Recovery Coach, you will additionally:

- Build recovery-oriented relationships grounded in hope, trust and respect
- Work collaboratively with participants to develop and review recovery goals




- Coach participants to build resilience, motivation, decision-making and practical recovery skills
- Support participants to navigate the NDIS and connect with mental health and community services
- Strengthen confidence, self-advocacy, social connection and independence

What we are looking for:

We are interested in the person behind the résumé as much as your qualifications.

You will ideally bring:

- Experience in NDIS Support Coordination, Psychosocial Recovery Coaching, Case Management, Mental Health or a related role
- Strong working knowledge of the NDIS, NDIA processes, plans, funding categories and service systems
- A sound understanding of psychosocial disability, recovery-oriented practice and person-centered support
- Strong communication and relationship-building skills, with the ability to work confidently with participants, families and stakeholders
- Strong case management, documentation, administration and computer skills, including Microsoft Office and client management systems
- A solution-focused, strengths-based approach and a genuine commitment to participant choice and outcomes
- Relevant tertiary qualifications in Social Work, Psychology, Mental Health, Allied Health, Community Services, Case Management or a related discipline are desirable
- A Cert IV in Mental Health with substantial relevant experience,



or equivalent skills and experience, will also be considered
- Current Australian Driver Licence and access to a reliable vehicle

Salary: $85,000 – $92,000 per annum

Employment: Full-Time, 12-Month Fixed-Term Contract

Travel: Participant and stakeholder meetings across the community.

Mandatory screening: Successful candidates will be required to hold or obtain a NDIS Worker Screening Check and Working with Children Check.
