The Patient Follow-Up Coordinator is responsible for the day-to-day administration and coordination of Remote Monitoring and Supported Self-Management Pathways (SSMP) within the Cancer Support Team. The post holder will manage patient activity across remote monitoring systems, ensuring accurate data entry, monitoring pathway compliance, undertaking quality assurance checks, identifying and escalating data gaps, and tracking patients throughout their follow-up journey.
Working closely with Cancer Support Workers, Cancer Nursing teams, Prehabilitation services and other multidisciplinary colleagues, the post holder will support the delivery of secure, effective and patient-centred follow-up care for people living with and beyond cancer. The role requires strong organisational skills, attention to detail, and the ability to work independently to manage competing priorities while maintaining high standards of data quality and patient confidentiality.
In addition, the post holder will provide comprehensive administrative and project support to the Cancer Support Team, including coordinating meetings, maintaining records, supporting service improvement initiatives, and acting as a key point of contact for patients and healthcare professionals involved in remote monitoring pathways’
