North West Sydney | Residential Aged Care

A well-established not-for-profit residential aged care provider in North West Sydney is seeking an experienced, people-focused Care Manager to join their team.

This is an excellent opportunity for a clinically capable and supportive leader who enjoys working closely with Facility Managers, RNs and care teams to deliver high-quality outcomes for residents.

You will join a medium-sized nursing home with an established leadership structure and further support from dedicated Quality and Education teams.

If you are an experienced Care Manager who leads through communication, consistency and clinical excellence, this role offers the opportunity to make a genuine impact within a respected not-for-profit provider.

About the Role

As the clinical leader supporting the home, you will provide direction to the RN and care teams, oversee clinical standards and work closely with the Facility Manager to ensure residents receive high-quality, person-centred care.

The role combines clinical leadership, team development, quality and compliance, and day-to-day operational oversight within a supportive and established environment.

Key Responsibilities

- Lead and support the RN and care team to deliver high-quality, person-centred care
- Oversee clinical care planning, documentation and medication management
- Monitor clinical indicators, incidents and risks, implementing appropriate actions where required
- Support the team to maintain compliance with the Aged Care Quality Standards




- Build strong relationships with residents, families and key stakeholders
- Coach, mentor and develop RNs and care staff to maintain high standards of care
- Work closely with the Facility Manager and Quality and Education teams on continuous improvement initiatives
- Support audits, accreditation preparation and the implementation of organisational policies and procedures

About You

You are a calm, approachable and clinically capable leader who genuinely enjoys supporting teams and improving outcomes for residents.

You will also bring:

- Previous experience working as a Care Manager within residential aged care
- Current AHPRA registration as a Registered Nurse
- Strong understanding of clinical governance and the Aged Care Quality Standards
- Confidence managing clinical issues, incidents, complaints and audits
- A supportive leadership style with the ability to coach and develop teams
- A genuine commitment to person-centred care and resident outcomes
