- Conduct comprehensive GP consultations for acute, chronic and complex conditions within a Chronic Care Team framework, in clinic and/or by telehealth.
- Deliver preventative health assessments covering cardiovascular risk, metabolic health, mental-health screening and lifestyle risk modification.
- Undertake chronic disease management reviews and care planning, including GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).
- Lead and participate in scheduled telehealth case conferences within multidisciplinary Chronic Care Teams.
- Integrate clinical, pathology, imaging and allied-health input into coordinated care plans.
- Initiate, review and manage pathology and imaging investigations.
- Provide prescribing and medication management.
- Refer patients to allied health, specialists and community services.
- Ensure clinical safety, prioritisation and escalation pathways are identified and actioned.
- Work with veterans and patients with complex, multi-system conditions across Medicare and DVA-funded care.
- Provide clinical leadership across in-clinic consultations and structured telehealth case conferencing.
Requirements
- Medical degree (MBBS or equivalent).
- Current registration with the Medical Board of Australia (AHPRA).
- Fellowship with RACGP or ACRRM (VR GP), or eligibility for a 19AB exemption in a DPA location such as Ipswich; international medical graduates welcome.
- Experience in chronic disease management, preventative health and complex care coordination.
- Competence in prescribing, referrals and investigation management.
- Solid communication and collaboration skills within multidisciplinary teams.
- Suitable home-office environment and reliable internet connection for the telehealth pathway.
- Experience with telehealth consultations and digital-health platforms is desirable.
- Experience within team-based or multidisciplinary care models is desirable.
- Interest or experience in veteran health, DVA processes or the CVC Program is desirable.
- Experience with GP Management Plans (GPMPs), Team Care Arrangements (TCAs) and chronic disease care planning is desirable.
