Primary Care Dietitians is seeking a Specialist Dietitian to lead a 12-month pilot service addressing frailty and malnutrition across care homes and community settings. You will work autonomously within the MDT, applying GLIM criteria and NICE guidance, prioritising food-first interventions and robust outcome measures.
The role is 0.6 WTE with a hybrid pattern (roughly 60% remote and 40% face-to-face). Must be HCPC registered, have 3+ years post-registration experience, and hold a full UK
