About your Role :

Client Assessment and Planning:

- o Conduct initial assessments to understand clients' needs, goals, and preferences.
- o Develop individualized care plans in collaboration with clients, their families, and other

Service Coordination:

- o Coordinate and oversee the delivery of various support services, including health, personal care, and community participation.
- o Ensure that all services are delivered in accordance with the NDIS guidelines and the client's care plan.

Case Management:

- o Act as the primary point of contact for clients and their families.
- o Monitor and review clients' progress and adjust care plans as needed.
- o Maintain accurate and up-to-date records of all interactions and services provided.

Support and Advocacy:

- o Advocate on behalf of clients to ensure they receive the necessary support and services.
- o Provide emotional and practical support to clients and their families.
- o Help clients navigate the NDIS system and access funding and resources.
- o Work closely with a multidisciplinary team, including healthcare providers, therapists, and social workers.
- o Facilitate communication and coordination among team members to ensure

Compliance and Reporting:

- o Ensure all services comply with NDIS standards and regulations.
- o Prepare and submit reports to NDIS and other relevant authorities as required.
- o Stay updated on changes to NDIS policies and procedures.

7. Quality Assurance:





- o Implement and monitor quality assurance measures to ensure high standards of care.
- o Gather feedback from clients and their families to improve services.
- o Participate in regular training and professional development activities.

Crisis Management:

- o Respond to emergencies and crises involving clients, providing immediate support and coordinating further action as needed.
- o Develop and implement risk management plans.

Budget Management:

- o Manage the budget for client services, ensuring efficient and effective use of resources.
- o Assist clients in understanding and managing their NDIS funding.

Community Engagement:

- o Foster relationships with community organizations and resources to support clients' integration and participation.
- o Organize and facilitate community activities and events for clients.

Training Management for Staff:

- o Develop and implement training programs to ensure staff are knowledgeable and skilled in delivering high-quality care.
- o Conduct regular training sessions and workshops for continuous professional development.
- o Monitor staff performance and provide feedback and support to enhance their skills and competencies.

Nursing Assistance:

- o Provide direct and comprehensive nursing care to a specific group of patients or clients with complex nursing care needs within a particular area of nursing practice.
- o Coordinate and ensure the maintenance of standards of nursing care for a specific group or population of patients or clients within a practice setting.
- o Coordinate or manage nursing or multidisciplinary service teams providing acute nursing and community services.
- o Provide leadership and role modelling, in collaboration with others including the Nurse Manager and Nurse Educator, particularly in areas of action research and quality assurance programs.

- o Oversee staff selection, management, development, and appraisal.
