Centacare is seeking an experienced Clinical Care Partner – Registered Nurse to join our Aged Care team in Darra, Brisbane South. This full time role supports older Australians to stay safe, healthy and independent at home, reporting to the Coordinator - Aged Care Services.
You will perform clinical assessments, care planning, wound care and medication management, coordinate care with families and multidisciplinary teams, and mentor Care Partners to ensure high-quality, person-centred care.
