Advanced Clinical Pharmacist–Department of Medicine of the Elderly

We have an exciting opportunity for an experienced pharmacist with a passion for the care of older people to join our team as Advanced Clinical Pharmacist–Department of Medicine of the Elderly.
This role will focus on providing expert clinical pharmacy support to elderly care services, working to optimise medicines use and improve outcomes for older patients with complex medical needs across Cambridge University Hospitals.
You will work closely with senior pharmacy colleagues, geriatricians, multidisciplinary teams and service leads to deliver and develop a high-quality clinical pharmacy service. You will have the opportunity to further develop your clinical leadership skills while leading projects focused on medicines optimisation, frailty, polypharmacy and improving patient safety across the elderly care pathway.
Liaise with the relevant senior pharmacy, medical and nursing staff, other allied health professionals (AHPs) and managers to promote safe, evidence based and cost-effective use of medication Department of Medicine of the Elderly (DME);
Provide an advanced clinical pharmacy service to designated wards as part of the pharmacy team
Prescribe medication for individual patients within the agreed scope of practice and according to the CUH pharmacist prescribing policy;
Participate in and lead where required on novel pharmacy projects, including management and growth of the discharge medicines service (DMS)
Support the education and training of Trainee Pharmacists, Rotational Pharmacists and Clinical Rotational Pharmacists working within the pharmacy medicine team;
Identify and undertake research to inform practice developments and underpin delivery of an effective, evidence-based pharmacy service;
Provide pharmacy leadership to Pharmacy Medicine team as a deputy for the Lead Pharmacists- Medicine
Deliver the additional duties described in Appendix 1
