Casual role
Pay rate –$47.66 per hour + casual loading + superannuation
Up to $18,550 tax free with not for profit salary packaging
- Travel will be reimbursed at 0.88 cents per km
We’re thrilled to announce that our Aged Care Assessment Services (‘ACAS’) team is expanding, creating exciting opportunities! ACAS positions offer flexibility and autonomy and are based across both metropolitan and regional locations, with most roles involving local travel within each region. We have an exciting opportunity for an Aged Care Assessor to join the team.
What will your role look like?
You will be conducting assessments to identify and meet the needs of clients and refer to appropriate home support services.
Day to day this includes:
- Collaboratingwith clients, families and multidisciplinary teams as required
- Working collaboratively with colleagues to promote a supportive and positive team culture
- Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determineclient needs and determiningeligibility for home support services
- Reviewing support plansto meet client’s changing support needs
- Working effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes
- Maintaining currency of knowledge relating to aged care services both funded and non-funded
- Assistingwith continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes
To be successful in this role you’ll need:
- An Australian Driver’s license and access to a registered, insured vehicle
- Excellent communication skills, both written and verbal
- A second language (Greek is desirable)
Commitment to excellence in customer service, both to internal and external stakeholders
Excellent planning, organising and problem-solving skills
Previous knowledge or work experience in the Aged Care System preferred
Completion of training requirements as specified under the My Aged Care workforce learning strategy 2023
Demonstrated understanding of and to undertake person centred, goal directed, strength based holistic assessments
Demonstrated understanding of work health safety principals including the required analytical skills to identify and effectively manage risks
Willingness to apply for a Police Check
