Conscious Care Group is seeking experienced Support Workers to join our boutique NDIS provider delivering Supported Independent Living (SIL) and Community Access Support (CAS) to participants with complex mental health needs.

You will provide person-centred support, follow care and behaviour plans, document shift notes and incidents, and work with the team to achieve protected outcomes across day and evening shifts.
