is seeking interested clinician Enrolled or Registered Nurse to join their team on this fixed term contract. This position will report to Manager.

The aim of the position is to provide support patients across their care continuum, working with patients & their care teams, identifying strategies or services to reduce the need to come to hospital.

The suitable candidate must;

- Excellent communication skills an ability to develop rapport with people quickly
- Be able to work independently but also collaboratively across care settings within a multi-disciplinary team
- Valuable Time Management
- Team Player
- Energic & passionate to achieve better health outcomes
- Motivate patients to work towards identified goals
