Omni-Care is seeking a compassionate Care Partner to join the Support at Home team. You will report to the Support at Home Manager, manage a caseload of clients, conduct home visits and care planning, and work with families, support workers and healthcare professionals to achieve client goals with high wellbeing.
You will ensure compliance with Aged Care Quality Standards and Support at Home program requirements while maintaining solid relationships with clients and stakeholders.
