Our client, Western Care Services (WCS), is a registered NDIS provider supporting people with disability to access services and supports that reflect their individual needs, choices and goals.

Main Duties/Responsibilities
- Coordinate complex support and services which are developed and delivered within the funding plan
- Develop and implement positive behaviour support plans
- Provide Specialist Support Coordination, Support Coordination and Support Connection
- Ensure service and support is delivered according to the standards and relevant policies and procedures plan and delivered according to the wishes of the individual and their families
- Maintain effective relationship and communication with internal and external stakeholders
- Maintain accurate documentation, preparing reports on individual outcomes and progress
- Build and maintain effective working relationships with individuals, families, carers and the community
- Consult with family members, professionals and other supports to ensure that support is ongoing and participants needs are identified and met
- Establish and maintain a current accurate confidential client reporting system
- Facilitate and support the development, maintenance and review plans as necessary
- Develop and maintain accurate information about individuals, i.e.) All About Me / Support Plans
- Develop responses to client issues in a timely manner
- Providing on-the-job guidance and mentoring,
especially in the implementation of behaviour support plans and managing challenging behaviours
- Provide a communication for participants and families and ensure that all relevant information is recorded and communicated
- Ensure reports and other required documentation are completed in accordance with Western Care Services requirements.
- Refer any concerns to the Operations Manager and assist in resolving
Requirements
Qualifications: Recognised qualification in Social Work and/or Developmental Education
Experience/Skills
- Membership with Australia Association of Social Workers (or willingness to obtain upon commencement)
- Experience and/or knowledge of providing support to people with disability
- Demonstrated ability to think innovatively and work flexibly to achieve goals
- Ability to develop, implement and manage plans and programs for individuals and groups
- Ability to self-manage work in a team environment
- Demonstrated strong problem solving and implementation skills
- Excellent communication and interpersonal skills with the ability to relate well to all people
