Better@Home is seeking interested clinician Enrolled or Registered Nurse to join their team on this fixed term contract. This position will report to Better@Home Manager.

The aim of the position is to provide support patients across their care continuum,



working with patients & their care teams, identifying strategies or services to reduce the need to come to hospital.

The suitable candidate must;

- Excellent communication skills an ability to develop rapport with people quickly
- Be able to work independently but also collaboratively across care settings within a multi-disciplinary team
- Positive Time Management
- Team Player
- Energic & passionate to achieve better health outcomes
- Motivate patients to work towards identified goals
