We're looking for an experienced and compassionate Support Coordinator who is committed to achieving positive outcomes for participants and their families.

About the Role

This is a dedicated Support Coordination role focused on helping participants understand, implement and maximise the benefits of their NDIS plans. The role is centred on delivering high-quality, participant-focused support, building robust relationships, and assisting participants to achieve their goals and access the services they need.

What You'll Be Doing
Supporting participants to understand and implement their NDIS plans
Coordinating services and connecting participants with suitable providers
Building strong relationships with participants, families, and stakeholders
Promoting independence, choice, and capacity building
Monitoring budgets and ensuring NDIS compliance
Preparing reports and supporting plan reviews




Advocating for participant goals and positive outcomes

What You'll Bring
Experience in NDIS Support Coordination and/or case management
Strong understanding of the NDIS and relevant frameworks
Excellent communication and relationship-building skills
Solid organisational and time management abilities
Qualifications in Disability, Social Work, Psychology, Allied Health, or a related field (desirable)
Confidence using Microsoft Office and case management systems

Essential Requirements
NDIS Worker Screening Check
Working With Children Check (WWCC)
National Police Check
Current Driver's Licence and reliable vehicle
