We are seeking an experienced and motivated NDIS Support Coordinator to join our growing team. This role is ideal for someone with a solid background in case management who is passionate about supporting people with disability to navigate the NDIS and achieve their goals.

As a Support Coordinator, you will work closely with NDIS participants, their families, and service providers to ensure participants receive the right supports, build capacity, and maintain choice and control.

Key Responsibilities
Provide Support Coordination in line with NDIS guidelines
Assist participants to understand and implement their NDIS plans
Coordinate and link participants with appropriate services and supports
Undertake comprehensive assessments, goal planning, and risk management
Monitor plan utilisation and support participants through reviews
Maintain accurate case notes, reports, and documentation
Advocate for participants to ensure their needs and goals are met
Build and maintain strong relationships with participants, families, and stakeholders

Essential Criteria
Minimum 1–2 years’ experience in case management (disability, community services, mental health, or related field)




Sound understanding of the NDIS framework and service system
Strong organisational and time management skills
Excellent written and verbal communication skills
Ability to work autonomously and manage a varied caseload
High level of professionalism, empathy, and problem-solving ability
NDIS Worker Screening Check (or willingness to obtain)
Working with Children Check (if applicable)
Current Australian Driver’s Licence and access to a vehicle

Desirable Criteria
Experience as an NDIS Support Coordinator or Specialist Support Coordinator
Relevant qualification (Community Services, Social Work, Psychology, Disability, or similar)
Experience working with complex needs and multiple stakeholders
