Life Without Barriers is seeking a Case Manager – Next Step Plus to support young people transitioning from out-of-home care on the North Coast. You will manage your own caseload with outreach-based, strengths-focused support across housing, education, employment, health, wellbeing and independent living.
You’ll coordinate services, advocate for clients and develop goal-focused plans, working with Child Safety and local partners. A current QLD driver licence is required.
