About the Role
The Practice Manager Service Navigation provides operational leadership across amultidisciplinary portfolio within Primary Care Connect (PCC). The role leads a central intake and service navigation function supporting all PCC programs and has oversight of programs including Chronic Disease Management, Allied Health and Community Connector initiatives.
Operating with a high level of autonomy, the Practice Manager is responsible for thedelivery, performance, and continuous improvement of integrated, client-centred, trauma-informed, and evidence-based services. The role ensures effective system navigation, so clients receive the right care, at the right time, in the right place, while aligning with organisational priorities and state and national policy directions.
Position Context
This position reports to the Executive Manager Health delivering a broad range of integrated primary health, prevention, and community-based services. PCC is committed to the social model of health, health equity, and improving outcomes for vulnerable populations.
The Practice Manager plays a critical role in managing service demand, coordinating care across multidisciplinary teams, and strengthening pathways into and between services,
and has operational oversight of a portfolio of multidisciplinary programs. The role requires close collaboration with internal programs, external partners, and community stakeholders to ensure seamless navigation and continuity of care.
Key Selection Criteria
The following criteria must be met and addressed in applications for consideration for this position:
Relevant tertiary qualifications in health, community services, practice management, or a related field.
Experience in the community health field with demonstrated leadership experience in health, community health or human services.
Strong leadership and people management skills with experience leading multidisciplinary teams and fostering a positive workplace culture.
Solid understanding of trauma-informed practice and experience working with vulnerable populations.
Strong understanding of service coordination frameworks and integrated care models, with the ability to design and improve client pathways and reduce barriers to access.
Experience in leading quality improvement initiatives, monitoring performance data, and ensuring compliance with accreditation standards, policies, and funding requirements.
Advanced communication, negotiation, and stakeholder engagement skills
Ability to interpret and use data to inform decision-making, monitor performance, and drive service improvements, including meeting reporting requirements.
Knowledge of community health principles and a commitment to providing high quality services in the community.
Commitment to client-centred, trauma-informed, and culturally safe practice.
Personal and professional alignment with PCC core values.
Experience working with vulnerable communities and understanding of culturallyresponsive service delivery.
Experience in service redesign or system improvement initiatives.
Understanding of the Social Model of Health and population health approaches.
