About the Role
The
Family Wellbeing and Referral Support Worker
is responsible for connecting young people and their families with the services, resources, and supports needed to improve wellbeing, strengthen family functioning, and achieve positive long-term outcomes.
This role has a strong focus on
service coordination, referrals, advocacy, and community engagement
, ensuring young people and their families can access the right support at the right time.
Working closely with the young person, family members, kinship networks, government agencies, and community organisations, the Family Wellbeing and Referral Support Worker will help address barriers relating to housing, health, education, employment, mental health, alcohol and other drugs, family relationships, cultural support, and community participation.
Key Responsibilities
Referral and Service Coordination
Act as the primary referral and service coordination contact for young people and their families.
Assess family and individual support needs and identify appropriate service pathways.
Develop and maintain strong relationships with local service providers, government agencies, Aboriginal organisations, and community groups.
Facilitate referrals to health, mental health, housing, education, employment, legal, financial assistance, family support, counselling, and cultural programs.
Support young people and families to attend appointments and engage effectively with services.
Advocate on behalf of young people and families to ensure access to required supports.
Monitor referral outcomes and follow up with service providers to ensure supports are being delivered.




Identify service gaps and barriers impacting young people and families and elevate concerns where required.
Family Support and Engagement
Build respectful, trusting, and culturally safe relationships with families and kinship networks.
Support families to actively participate in planning and decision-making processes.
Assist families to navigate complex service systems and government programs.
Provide practical support and information to strengthen family functioning and wellbeing.
Facilitate family meetings and support family-led decision-making.
Encourage positive family relationships and communication.
Work collaboratively with young people to identify goals and support needs.
Assist young people to develop life skills, community connections, and support networks.
Support young people transitioning from the Maruma-Li Walaay program back into family, education, employment, training, or community settings.
Promote engagement with culturally appropriate services and activities.
Encourage positive participation in education, training, recreation, and community programs.
Community and Stakeholder Engagement
Establish and maintain effective working relationships with:
Youth Justice
Schools and educational providers
Aboriginal Medical Services
Family and Community Services
Health and mental health providers
Housing and homelessness services
Employment and training providers
Local Aboriginal organisations
Community and cultural groups
Participate in networking meetings, case conferences, and interagency forums.
Strengthen partnerships that improve referral pathways and outcomes for young people and families.

Work collaboratively with Elders and cultural leaders where appropriate.

Ensure cultural considerations are incorporated into support planning and referrals.
Documentation and Reporting




Maintain accurate case notes, referral records, service plans, and outcome reports.
Track referral activity and service engagement outcomes.
Prepare reports and updates for management and funding requirements.
Ensure confidentiality and privacy requirements are met.
Participate in supervision, team meetings, and continuous improvement activities.
About You
You are passionate about supporting young people and families and have a strong understanding of how to navigate community support systems.
Experience in community services, family support, case management, youth work, social services, or a related field.
Demonstrated experience coordinating referrals and supporting clients to access services.
Strong knowledge of local community services and support systems.
Understanding of Aboriginal culture and commitment to culturally safe practice.
Ability to build positive relationships with young people, families, community organisations, and government agencies.
Excellent communication, advocacy, and stakeholder engagement skills.
Experience working with individuals and families experiencing complex needs.
Strong organisational skills and ability to manage multiple referrals and support plans.
Current NSW Driver Licence.
Current Working With Children Check (WWCC).
National Criminal History Check (or willingness to obtain).
Qualification in Community Services, Social Work, Welfare, Counselling, Youth Work, or a related discipline.
Experience working with Aboriginal communities and families.
Knowledge of youth justice, child protection, family preservation, or bail support programs.
Experience facilitating case conferences and multidisciplinary support planning.
Existing relationships with local service providers within the Moree and North West NSW region.
