Speech Pathologist, Transition Care Program

The Team & Role
Join Bayside Health's Transition Care Program (TCP), a multidisciplinary service supporting older adults to transition safely from hospital to home across the Mornington Peninsula. As the Grade 2 Speech Pathologist, you will manage a diverse community-based caseload, working closely with Nursing, Physiotherapy, Occupational Therapy, Dietetics and Social Work to deliver evidence-based, person-centred care. While this is a sole clinician role within TCP, you will be professionally supported by a broader Speech Pathology team committed to clinical excellence, continuous improvement and student education.
Skills & Experience Required
Eligible for practising membership of Speech Pathology Australia.
Sound experience in the provision of best/evidence based adult related speech pathology.
Strong communication and partnership skills with clients, families/carers and health professionals.
Well-developed communication and interpersonal skills
Knowledge of / experience in a community settings.
