About the role
To contribute to and be part of the Subacute at Home Program team to support patients and carers in their homes to achieve an optimum level of health independence through the provision of patient- focused holistic interventions to those assigned to him/her, in order to achieve desired clinical outcomes.
The role involves working as a high-functioning member of the Subacute at Home Program at Barwon Health, providing high quality, efficient, evidence-based physiotherapy services via:
Provision and development of Physiotherapy service delivery within Subacute at Home, applying comprehensive, patient-centred care within an interdisciplinary setting.
Provision of teaching, supervision, support and development of less experienced staff, allied health assistants (as required) and students.
Manage designated quality improvement projects within the expectations of the Barwon Health Strategic Plan and the Subacute at Home Business Plan.
Key responsibilities
Provide high quality clincial interventions to Subacute at Home Program patients.
Support and develop less experienced staff, allied health assistant and students through clinical guidance, education, and fostering evidence-based practice.
Practice in accordance with National Safety and Quality Standards, contributing to continuous improvement and safe care delivery.
Collaborate effectively with the interdisciplinary team to support staff performance, professional development, and high-quality patient outcomes.
Whatwe'relooking for
Degree in Physiotherapy from an Australian Physiotherapy Council accredited course
Current national registration as a Physiotherapist with the Australian Health Professionals Regulation Agency
Demonstrated effective communication and interpersonal skills including the ability to develop and maintain effective working relationships with colleagues and other health service providers, conflict resolution, influencing others, and facilitating open discussions.
Excellent communication and interpersonal skills, including the ability to build effective working relationships and influence outcomes
Strong organisational, prioritisation and time management skills
