About the Role
Reporting to the General Manager, you will provide clinical leadership across our community nursing and aged care services.
Your key responsibilities will include:
Managing a portfolio of clients and overseeing their clinical care and support.
Conducting comprehensive assessments and developing individualised care plans.
Providing clinical advice, escalation support and mentoring to nurses and care teams.
Leading
wound care
, including complex assessments, treatment plans and reviews.
Supporting clinical care across wound care, catheter care, continence, diabetes, seizures and medication support.




Coordinating nursing referrals, assessments and clinical services.
Reviewing clinical documentation and supporting compliance with regulatory requirements.
Managing clinical incidents, investigations and improvement actions.
Educating and mentoring team members to strengthen clinical capability.
Building strong relationships with clients, families, healthcare professionals and community stakeholders.
About You
You are an experienced
Registered Nurse and confident clinical leader
who enjoys working in community care and supporting others to deliver exceptional outcomes.
You will have:
Current
AHPRA Registered Nurse registration
.
Demonstrated clinical leadership and people management experience.
Experience in community nursing, aged care, disability or acute care.
Strong clinical assessment, documentation and care planning skills.
Wound care and complex wound management experience.
Experience mentoring and educating clinicians.
Knowledge of aged care, Home Care Packages and community-based services.
Excellent communication, organisation and stakeholder management skills.
Ability to work autonomously and collaboratively.
Current Police Check, First Aid, CPR and NDIS Worker Screening Check, or willingness to obtain.
Current Australian Driver's Licence and reliable vehicle.
