Key Responsibilities
Manage a caseload of NDIS participants, delivering high-quality Support Coordination and/or Psychosocial Recovery Coaching.
Monitor participant budgets to ensure funding is used effectively and within plan limits.
Coordinate, review and oversee service delivery to ensure supports meet participant goals and quality expectations.
Advocate for participants and support them to exercise choice, control and access to appropriate services.
Build robust relationships with participants, families, providers and key stakeholders.
Assist participants to navigate the NDIS, prepare for plan reviews and maximise plan outcomes.
Identify and respond to risks, safeguarding concerns and changing participant needs.
Maintain accurate documentation, case notes and reports in line with NDIS Practice Standards and organisational requirements.
Contribute to a collaborative team culture focused on quality outcomes and continuous improvement.
Identify, manage and escalation participant risks or concerns to senior management as required.
Attend and actively participate in team meetings, supervision and professional development.
About You

To be successful in this role, you will have:
Experience as an NDIS Support Coordinator, Recovery Coach or in a similar community services role.
Sound knowledge of the NDIS, Psychosocial Recovery Coaching and person-centred practice.
Excellent communication, relationship-building and advocacy skills.
Strong organisational and time management skills, with the ability to manage participant budgets and competing priorities.
Confidence working independently while contributing positively to a collaborative team.
High attention to detail and the ability to maintain accurate, compliant documentation.
To be successful in this role, you will have:
Demonstrated experience in mental health, disability, Support Coordination and/or Psychosocial Recovery Coaching.
Experience managing NDIS participant plans, budgets, goal setting and recovery-focused supports.
Strong understanding of the NDIS, trauma-informed practice and recovery-oriented approaches.
Excellent communication, advocacy, conflict resolution and problem-solving skills.
Strong organisational skills with the ability to manage competing priorities and a diverse caseload.
Current Victorian Driver's Licence and access to a reliable, insured vehicle.
Current NDIS Worker Screening Check (or willingness to obtain).
Current Working with Children Check (or willingness to obtain).
National Police Check (or willingness to obtain).
Current First Aid and CPR certification (or willingness to obtain).
Desirable:
Qualification in Mental Health, Community Services, Social Work, Psychology or a related discipline.
Experience supporting participants with complex psychosocial, behavioural or disability support needs.
