Austin Health's Continuing Care Service comprises the following Divisional specialties and services, which are provided within a range of care settings including inpatient, outpatient and community -based locales across Austin Health's three key campuses:

Acquired Brain Injury Unit
Spinal Cord Rehabilitation Service

About the role
The Continuing Care Project Manager is responsible for providing expert improvement advisory services across the Continuing Care department, to support and drive measurable and sustainable improvements in patient access, experience and outcomes.
They are responsible for delivering this by building divisional capability through delivery of training and education in improvement science, engagement of staff through the Improvement Network, advice and coaching to staff members leading improvement work, and direct leadership of priority divisional improvement initiatives.
About you
You will be someone who has excellent communication skills and strong commitment to improvement and project work resulting in excellent patient outcomes.
To be successful in this role you'll bring with you:
Track record leading complex, improvement and transformation initiatives, delivering measurable clinical, operational and financial outcomes
Ability to analyse complex datasets, quantify opportunities, model impact, and track benefits realisation to inform decision-making and drive performance improvement
Understanding of healthcare service delivery,



performance drivers, and operating models within large, complex health systems
Capability in applying improvement methodologies (e.g. Improvement Science, Lean) at scale to achieve sustained change
Experience in engaging staff and embedding sustainable improvement capability through coaching, training and structured programs
Strong project management skills, with capability in setting direction, prioritising work, navigating ambiguity, managing risks, and driving delivery against agreed timelines and performance targets
