Santa Clara Home Care is seeking an Accredited Exercise Physiologist for a casual, mobile in‐home role across Greater Sydney.
You will assess function and falls risk, prescribe and progress exercises to help older people regain strength, balance and independence at home, and work as part of a multidisciplinary team.You will lead goal‐based restorative and reablement programmes under the Support at Home pathway, coach clients and families, and maintain explicit notes.
