Your day-to-day will include:Providing proactive, personalised support to patients and carers navigating their cancer care pathwayCoordinating appointments, referrals, and communication across clinical teams to reduce gaps and delaysIdentifying and connecting patients with psychosocial, financial,



and community support servicesMaintaining accurate case records and contributing to team reviews to improve care coordination outcomesWhat You'll BringA background in nursing, allied health, or a related clinical discipline with current AHPRA registration or relevant qualificationsExperience in oncology, chronic disease management, or complex care coordination settingsDemonstrated ability to communicate sensitively with patients facing responsible illness and their support networksStrong organisational skills with the ability to manage competing priorities across a multidisciplinary environmentFamiliarity with electronic medical records and care coordination toolsAn understanding of the psychosocial dimensions of cancer care — something central to the Lifehouse modelRegistration with the Australian Health Practitioner Regulation Agency (AHPRA) - if applicableLegal right to work in Australia - we regret no sponsorship is offered for this positionCurrent Working with Children Check (WWCC)National Police Check (within the last 6 months or willingness to obtain)Compliance with Category A immunisation requirementsA Workplace That Welcomes EveryoneAt Chris O'Brien Lifehouse, we are committed to a culture of inclusion and respect.
