The Role
We are looking for an experienced Part time Chronic Disease Management (CDM) Nurse to join our Forest Hill practice and be a driving force and advocate for preventative health within the local community.
This role will see you support GPs and their patients to identify and manage chronic conditions, helping patients improve their health outcomes and ultimately stay out of hospital.
The Tasks




Prepare Care Plans and Team Care Arrangements to help patients manage their conditionsEmpower patients to take greater ownership of their healthCoach patients on embedding positive health behaviour changesEnable patients to access ongoing care from a multidisciplinary teamAssist with the early identification of chronic disease by conducting health assessmentsWork with GPs to promote preventative healthcare throughout the practiceIncrease the number of patients receiving a care plan for the first timeEnsure patients' care plans are reviewed regularlyManage administrative tasks relating to CDM appointments
The Successful Candidate
AHPRA-registered Enrolled Nurse or Registered NurseProven experience in Chronic Disease Management and health assessmentsStrong knowledge of health assessments is highly regardedExcellent organisational and time management skillsOutstanding communication and customer service skillsA passion for helping people and a personable approach
