Every role at City of Parramatta contributes to this next chapter, helping us build a robust, thriving Parramatta for the people who live, work and spend time here.About the roleThis position is responsible for delivering best practice support to individuals and groups across Community Care focused on people over 55 years, people with disabilities and carers.In this role you will:Deliver flexible, participant‐centred support across individual and group activities to enhance health, wellbeing, and social connection, while meeting KPIsManage and coordinate caseload, including assessment, planning, service delivery, referrals, and exitsFacilitate group activities in an enabling way, empowering participants to take ownership and reduce reliance on formal supportsSupport and guide volunteers to deliver quality servicesManage and resolve day‐to‐day issues, escalating where requiredWhat's on offerWe are offering a full-time permanent opportunity with a competitive remuneration package from $73,****** to $80,****** plus 12% superannuation.Your employee experience with City of Parramatta includes:Flexibility: Hybrid working conditions, flexi‐time scheme, additional leave over the Christmas periodLeave:
