Communify is seeking a Pathways Facilitator to join the multidisciplinary team at our Medicare Mental Health Centre in Redcliffe.

You will provide a calm, empathetic and trauma-informed response while coordinating referrals and maintaining accurate service information.Your key responsibilities will include:Responding to enquiries from participants, carers, families, referrers and stakeholders by telephone, email and textBuilding positive rapport and helping people navigate available services and supportsReceiving, assessing and coordinating incoming referrals through Redicase, PMHCIS, email,
telephone and faxResponding appropriately to people who may be experiencing mental health challenges or distressMaintaining accurate participant notes, referral information, databases and reporting recordsSupporting the Centre to meet service targets and reporting requirementsCoordinating group programs, including scheduling, administration, monitoring and data collectionProviding general administrative support, including scanning, data entry, calendar management and correspondenceMonitoring and ordering office supplies and resourcesWorking collaboratively with the multidisciplinary team to support a responsive and welcoming serviceAbout youYou will bring warmth, sound judgement and excellent organisational skills to a busy participant-facing environment.
You will be comfortable communicating with people experiencing complex or challenging circumstances and able to balance competing priorities without losing attention to detail.To succeed in this role, you will have:Relevant qualifications and/or experience in community services, mental health or a related sectorDemonstrated experience in administrative practices, systems and processesExcellent interpersonal and communication skills across telephone, email, written and face-to-face interactionsThe ability to communicate calmly, confidently and empathetically with people experiencing mental health challengesStrong planning and organisational skills,
including the ability to manage competing priorities and meet deadlinesThe capacity to respond flexibly and constructively to uncertainty and changing circumstancesStrong digital literacy and experience using Microsoft Office applicationsExperience with client management, referral or health information systems would be highly regardedThe successful applicant will be required to hold or obtain:A current open Queensland driver licenceA valid Working with Children CheckA valid NDIS Worker Screening CheckA satisfactory National Police CheckAbout CommunifyCommunify supports people across all stages of life to lead active, healthy and socially connected lives.We deliver services across mental health, aged care, disability, housing and homelessness, children and families, drug and alcohol recovery, and the NDIS.
