This is a casual position that offers flexibility whilst making a meaningful impact on the lives of individuals we support.
As Day Program Team Leader, you will play a crucial role in overseeing daily program activities, ensuring participants receive engaging, person-centred support, and leading a team to deliver quality outcomes.
This role is integral to our aged and disability support services, helping us maintain our commitment to empowering individuals and enhancing their wellbeing through structured, purposeful day programs.Key responsibilitiesLead and supervise day program activities, ensuring they are engaging, inclusive, and tailored to participant needs and preferencesCoordinate with team members to deliver high-quality support and care throughout the program dayAssess participant wellbeing and monitor engagement, reporting on outcomes and any concernsImplement individual care plans and support strategies in accordance with participant goals and organisational policiesCreate and maintain a safe, welcoming and stimulating environment for all participantsManage program schedules, resources, and equipment effectivelyCommunicate with families, carers,



and relevant stakeholders regarding participant progress and program activitiesMaintain comprehensive records and documentation as required by legislation and organisational standardsSupport the professional development and wellbeing of team membersEnsure compliance with workplace health and safety requirements and organisational proceduresWhat we're looking forExperience in community services, aged care, or disability support settings with demonstrated leadership or supervisory experienceStrong understanding of person-centred care principles and commitment to delivering individualised supportExcellent communication and interpersonal skills with the ability to work effectively with participants, families, team members, and stakeholdersDemonstrated ability to lead, motivate, and support team members in delivering quality careSound knowledge of relevant legislation, including the Disability Services Act and aged care standardsCurrent First Aid and CPR certification, or willingness to obtain these qualificationsValid Working With Children Check and/or National Police Clearance as requiredStrong organisational and time management skills with attention to detailAbility to respond compassionately to challenging situations and resolve conflicts constructivelyPhysical capability to support activities and mobility assistance as requiredRelevant qualifications such as Certificate III or IV in Disability Support, Aged Care, or Community Services are highly valuedWhat we offerHeart and Home Care is committed to supporting the wellbeing and professional growth of our team members.
