service providers and internal teams, you'll coordinate tailored supports that enable older people to live safely and independently at home while achieving meaningful, person-centred outcomes.Key Responsibilities:Build strong, trusted relationships with clients and their familiesConduct comprehensive assessments and identify individual goals and support needsDevelop, implement and regularly review person-centred care plansCoordinate services and referrals to ensure holistic and seamless supportSupport clients to make informed choices regarding services and fundingMonitor client wellbeing and proactively identify changing needs and risksCollaborate with nurses, allied health professionals, GPs and community providersSupport restorative and wellness-focused approaches that maximise independence and functionManage a diverse caseload while maintaining accurate and compliant documentationRespond to client feedback, concerns and incidents professionally and empatheticallyAbout you:You are passionate about supporting older people to live independently, safely and with dignity in their own homes.
You bring a person-centred approach to care, enjoy building meaningful relationships, and are confident working with clients, families and health professionals to achieve positive outcomes.We're looking for someone who has:Experience working within the Aged Care, Community Services or Health sectors, with a strong understanding of client wellbeing, safety and quality careDemonstrated experience in the delivery of aged and community care services,



supporting older people to achieve their goals and maintain independenceExperience working within a clinical, nursing or allied health environment, with the ability to apply professional knowledge to client care planning and service coordinationCase Management or Care Management experience, including the coordination of supports for clients with diverse and changing needsExperience conducting assessments, care planning, implementation, monitoring and evaluation of services, with accurate and timely documentationA robust commitment to person-centred care and Consumer Directed Care principles, ensuring clients remain at the centre of all decisionsExcellent communication and relationship-building skills, with the ability to engage effectively with clients, carers, service providers, treating health professionals and the broader communityThe ability to develop and coordinate needs-based, consumer-focused services that support positive client outcomesA sound understanding of compliance requirements, including safety, privacy, equal opportunity and professional responsibilities within a care environmentStrong organisational skills, the ability to manage competing priorities and maintain high-quality recordsThe ability to work autonomously while thriving as part of a collaborative and supportive teamAre committed to being part of a workplace culture based on the BESTway of life (Believe, Excel, Support and Trust)Essentials for working in Aged Care:Applicants must have, or be willing to obtain:A current Victorian Driver Licence and access to a reliable, insured vehicleCurrent First Aid and CPR certificationWorking with Children Check (Employee)National Police CheckUp-to-date vaccinations in accordance with Uniting AgeWell's Vaccination PolicyFull Australian working rightsAboriginal and Torres Strait Islander peoples are encouraged to apply.
