SW4 Classification: $92,437 to $99,452 pro rata
SW5 Classification: $105,736 to $110,518 pro rata

Cafs is seeking dedicated and compassionate practitioners to join our Youth Services team as Targeted Care Packages (TCP) Key Workers, supporting children and young people with complex needs to achieve greater stability, connection and wellbeing.

About the Program
Targeted Care Packages (TCPs) provide flexible, individualised supports to children and young people who are in, or at risk of entering, residential care.
Working closely with children, families, carers, Child Protection and community services, TCPs help create secure, nurturing and sustainable care arrangements that improve long-term outcomes.
About the Role
As a Targeted Care Packages Key Worker, you will:
Provide case management and care coordination for children and young people with complex needs.
Build strong relationships with children, families, carers and key stakeholders.
Develop and implement support plans aligned to each young person's goals and best interests.
Promote placement stability, connection to family, education and community.
Deliver culturally responsive, trauma-informed and therapeutic practice informed by TBRI.
Work collaboratively with Child Protection, carers, schools and service providers to achieve positive outcomes.
SW4 Opportunities
The SW4 role is ideal for practitioners who enjoy direct case management and working collaboratively within a multidisciplinary team to support children and young people to thrive.
SW5 Opportunities
The SW5 role is suited to experienced practitioners who can manage more complex cases, provide guidance to colleagues and contribute to high-quality practice across the program.
