Bayside Health in Frankston is seeking a Speech Pathologist to join the Transition Care Program on a permanent part time basis (48 hours per fortnight). You will manage a diverse community-based caseload, collaborating with Nursing, Physiotherapy, Occupational Therapy, Dietetics and Social Work to deliver evidence-based, person-centred care.
