Working as part of a multidisciplinary team, you will support patients throughout their healthcare journey by providing advocacy, health education, cultural support, and care coordination. Aboriginal Health Practitioners will also have the opportunity to deliver direct clinical care within a delegated model of care and scope of practice. You will work collaboratively with clinicians, Aboriginal Hospital Liaison Officers, and community partners to ensure Aboriginal people receive high-quality, culturally appropriate healthcare and support.

What you will bring to the role

- Must be of Aboriginal or Torres Strait Islander descent (an applicant's race is a genuine occupational qualification and is authorised by Section 14 of the Anti-Discrimination Act 1977, NSW)



and a demonstrated ability to effectively and sensitively liaise and communicate with local Aboriginal and Torres Strait Islander people and communities and agencies.
- Has undertaken or is willing to undertake, a Certificate IV Aboriginal Primary Health Care Practice with current registration as an Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency.
- Demonstrated ability to work in a multidisciplinary team and liaise with other health professionals and external agencies to facilitate integrated care for Aboriginal people.
- Demonstrated understanding of the issues impacting the health of Aboriginal people, particularly mental health, drug & alcohol, psychosocial, domestic violence or sexual assault issues.
- Must be able to identify and implement health promotion activities within a secure workplace for Aboriginal and/or Torres Strait Islander Patients.
- Demonstrated oral and written communication skills, time management, and computer literacy, including Microsoft Office programs
- Ability to work unsupervised, take initiative, and take a lead role in initiating and carrying out community development activities.
